Iodine in the treatment of hyperthyroidism

Ross DS

"Mechanism of Action — Pharmacologic amounts of iodine ameliorate hyperthyroidism by blocking thyroid hormone release. In addition, there is abnormal autoregulation of iodine economy in these patients. In normal subjects, the administration of pharmacologic amounts of iodine leads to temporary inhibition of iodine organification in the thyroid gland, a phenomenon called the Wolff-Chaikoff effect; escape eventually occurs, allowing thyroid hormone synthesis to proceed. In autoimmune thyroid disease, however, iodine-induced blockade of iodine organification persists and can result in, or exacerbate hypothyroidism in patients with Hashimoto's thyroiditis, or ameliorate hyperthyroidism in Grave's disease. This effect is even more pronounced after radioiodine treatment of Graves' hyperthyroidism."
